It's objectives are: To improve communication and linkages with primary care providers. To build upon existing programs to improve services available for the management of chronic disease. To improve access to and utilisation of tools and strategies that will assist in better managing patients with chronic disease. To develop local chronic disease care pathways and referral tools to better manage patients with chronic disease. To engage with other primary care initiatives that promote integrated primary and community care. To coordinate and facilitate education training dependent on primary health care needs.
