Promoting solutions to improved integration in the management of chronic disease

About the Program (  Factsheet)

The Primary Care Integration program aims to promote solutions to improved integration of General Practice and other local health providers in chronic disease management. It's objectives are:

  • To improve communication and linkages with primary care providers.
  • To build upon existing programs to improve services available for the management of chronic disease.
  • To improve access to and utilisation of tools and strategies that will assist in better managing patients with chronic disease.
  • To develop local chronic disease care pathways and referral tools to better manage patients with chronic disease.
  • To engage with other primary care initiatives that promote integrated primary and community care.
  • To coordinate and facilitate education training dependent on primary health care needs.
